This 26-year-old female came to us with a history of a previous rhinoplasty by another physician. She was displeased with the external appearance and presented for surgical revision. Physical examination revealed left sided nasal obstruction due to septal deviation and inverted “V” deformity (The shadow from a narrow cartilagenous dorsum accentuating the edge of the nasal bones). Aesthetically, she also had a bulbous tip, hanging columella with increased nostril show, a deviation to the left with general asymmetry and an under-rotated tip with a long nose.
When comparing her frontal before and after photos, notice how her dorsum is no longer slanted to the left, giving her a much improved symmetry. The tip is much more symmetric and the nose is shorter. Her lateral and oblique photos show the most significant improvement, her long appearance has been corrected by altering her hanging columella and rotating the tip. The base view also demonstrates the significant improvement in symmetry (The incision will continue to improve over the year). Doing this procedure has given the patient more confidence but overall a more youthful appearance.
